I'm trying to find documentation that states how we setup the controller for Database monitoring when dealing with a SQL Server instance (myserver\myinstance). Does myserver\myinstance go into the hostname? Thanks

I was able to do this by specifying the JVM arguments when installing the service as per https://docs.appdynamics.com/display/PRO45/Install+the+Database+Agent+as+a+Windows+Service

You have to download the Windows specific zip from https://download.appdynamics.com/download/#version=&apm=db%2Cappd4db&os=&platform_admin_os=&appdynamics_cluster_os=&events=&eum=&page=1 and it will contain the required files.
